Анотацiя В роботі висвітлені проблеми і труднощі, з якими стикаються пацієнти з епілепсією, що досягли повноліття, та їхні батьки під час зміни медичної установи і лікуючого лікаря через перехід до служби охорони здоров’я, орієнтованої на дорослих. Обговорюються чималі клінічні та психосоціальні ускладнення несприятливого перебігу епілепсії у дорослих, які захворіли в дитинстві. Подані моделі еволюції дитячої/підліткової епілепсії, які залежать від причини захворювання. Наведені приклади організації плавного і безпечного переходу пацієнтів з епі- лепсією, які захворіли в дитинстві, під спостереження неврологів/епілептологів для дорослих у різних країнах світу. Наголошується, що ключем для створення індивідуалізованої програми успішного переходу є врахування етіології епілепсії, особливо якщо молоді пацієнти потребують спеціального догляду в зв’язку зі зниженням інтелекту. Розглянуто можливості такого переходу в Україні на прикладі роботи Львівського обласного протиепілептичного центру.
